If you are being evaluated for an insurance covered procedure please bring your photo ID and insurance card to your visit. Check with your insurance carrier for any applicable co-payments required. Co-payments are due at the time of the appointment. For most insurance covered procedures, we are required to provide clinical information to your insurance carrier and request authorization- a process known a pre-determination. Depending on the individual company this may take 1 to 8 weeks to complete.
Combined Pre-paid and Insurance Procedures
Patients scheduled for a cosmetic pre-paid procedure in conjunction with a medically necessary procedure will have the medically necessary portion billed to their medical insurance. Although prior authorization may have been obtained for your medically necessary surgery, it is not a guarantee of payment. It is important for you to understand as much as possible about your particular insurance plan. Please check with your insurance carrier for specific information related to your benefits. Understanding your coverage will put you in a better position to make decisions about your medical care. Your medical procedure may be subject to deductibles, co-payment, coinsurance and/or non-covered services. MetroHealth Medical Center may contact you regarding your out-of-pocket portion of the medically necessary surgery and may require a pre-payment or payment arrangements before your surgery takes place. Please note that the pre-payment for your elective cosmetic portion of you procedure does not cover and is completely separate from any deductibles or co-payments required by your carrier for the medically necessary portion of your surgery.
Payments and Financing
Payment for your procedure(s) may be made by cash, bank check, or credit card and is due 2 weeks prior to the date of your surgery. In the event that you cancel your surgery, a full refund will be issued to you less the original consultation fee which is not refundable.
Credit Cards Accepted
We do participate with this financing program. Please check with the office for current payment plans that we accept. For more information visit www.carecredit.com
Complications that require a return to the operating room or medical therapy following elective cosmetic surgery are rare but can occur. Costs associated with additional treatment are unlikely to be covered by your insurance company and are the patient's responsibility. I participate in the CosmetAssure® program which provides you with a short-term insurance that covers costs associated with additional treatment resulting from a complication. This service is available for most cosmetic procedures. We will provide you with more information at your consultation. For more details now please visit www.cosmetassure.com.
Please notify our office 24 hours prior to your appointment if you need to cancel. If you are more than 30 minutes late for your appointment you may be asked to reschedule to another date.
We charge a $65 fee for new cosmetic patient evaluations. Payment is made at the time of scheduling your appointment. This consultation fee covers your initial evaluation (and a second office if necessary) and will be applied to the cost of surgical procedure scheduled within 6 months of the initial visit. New patients to our practice are required to present photo identification at the time of the first visit.
